If you are married, there are two major that you must complete in order to apply for loans and bursaries. These forms are forms 1001, 1152, and 1005 (if you have children).

Please note that students Married must also submit the following supporting documents:

*All supporting documents must be copied completely and brought to Services Quebec to be stamped by a commissioner of oaths*

• PR card (both sides) or confirmation of permanent resident paper

• Birth Certificate Translation

• CSQ

• Marriage Certificate Translation

• Form 1152

  • Form 1076 (if spousal income not declared in Quebec is greater than 0)

• Form 1005 (if you have dependant children)

  • Children's Birth Certificates
