If you are single, there are two major forms that you must complete in order to apply for loans and bursaries. These forms are forms 1001 and 1095.

• Form 1001 (main application form for 2015-2016 academic year)

• Form 1095 (declaration that parents do not reside in Canada)

 

Please note that students who are Single (not married) must also submit the following supporting documents:

*All supporting documents must be copied completely and brought to Services Quebec to be stamped by a commissioner of oaths*

• PR card (both sides) or confirmation of permanent resident paper

• Birth Certificate Translation

• CSQ

• Form 1095
